Over the coming months, you’ll learn how to help yourself achieve a healthier diet and lifestyle, implementing simple habits step by step you’re able to stick to long term.

This Agreement is made between the Coach of the Program (Elise Sproll) and you (the Client). The Program you have joined includes:

• A 4-week online course (if you have already attended this course, a 20% discount will apply to your overall program cost)

• A 1-hour fortnightly session for the duration of your program (4-months or 6-months) to discuss your progress and my recommendations

• A variety of handouts, recipes, and other materials

• Text or email support between sessions

SCHEDULING

I understand my clients have busy schedules and I take pride in not keeping them waiting or keeping them longer than planned. Each session will end 1-hour after it was scheduled to begin. Please be on time. If you need to cancel or reschedule the appointment, you must provide 24 hours' notice, or you will forfeit that appointment and not have an opportunity to reschedule it. All reschedules must be made within that same fortnight, so as not to push out the original end date of the client’s program. The coach will consider a program pause or movement of session end date on a case-by-case basis, dependent on unforeseen circumstances.

Your program start and end date are in your confirmation email. This program expires if all sessions have not been completed within two months after the End Date specified above.

SATISFACTION GUARANTEE

I have a satisfaction guarantee because I’m committed to helping clients get well. But, if the client finds my approach isn’t working within the first 30 days, I’ll end the program without a cancellation fee. If the client has paid upfront in full, I will repay the remaining portion of the program (e.g. 3 months of 4 months).

PAYMENTS AND REFUNDS

The cost of my 6-month program is $2900 including GST, paid over 6 months or $2500 including GST if paid upfront in full. The cost of my 4-month program is $2000 including GST, or $1600 if paid upfront in full. If the client is already receiving a 20% discount as a past group coaching course attendee, the upfront payment discount will not apply. The client will receive a 20% discount on my 4-month or 6-month program if they have already joined the Cancer Confidence Series or Feel Better Series.

Payments are made online or can be debited into the account listed on your invoice. Payment options are available through the payment portal on your invoice.

All payments made to Own Your Health are non-refundable except for valid 30-day satisfaction guarantee requests. Month 1 payment (or paid in full total if this is what the client has agreed) must be paid before commencing the first coaching session.

In the event of the Client’s absence or withdrawal, the client will remain responsible for the pro rata share of the program that has been delivered, plus a cancellation fee of $200. If the client decides to cancel the program before it begins (but after sessions have been agreed and scheduled), the cancellation fee will need to be paid.

I reserve the right to cancel the program if at any point I feel it is not advantageous to continue. If this happens, you are only responsible for the pro rata share of coaching services received. This is because I am looking for clients who are as committed to their health transformation as I am. I take my role as a coach seriously, and maintain a high level of positivity, encouragement and support - but most importantly, I’m results-orientated. If you aren’t able or willing to implement the changes we identify and agree, I can’t help you.

DISCLAIMERS

The Client understands that the role of the Health Coach is not to prescribe or assess micro and macronutrient levels; provide health care, medical or nutrition therapy services; or to diagnose, treat or cure any disease, condition or other physical or mental ailment of the human body. Rather, the Coach is a mentor and guide who has been trained in holistic health coaching to help clients reach their own health goals by helping clients devise and implement positive, sustainable lifestyle changes. The Client understands that the Coach is not acting in the capacity of a doctor, licensed dietician-nutritionist, psychologist or other licensed or registered professional, and that any advice given by the Coach is not meant to replace advice by these professionals. If the Client is under the care of a health care professional or currently uses prescription medications, the Client should discuss any dietary changes or potential dietary supplements use with his or her doctor and should not discontinue any prescription medications without first consulting his or her doctor.

The Client has chosen to work with the Coach and understands that the information received should not be seen as medical advice and is not meant to take the place of seeing licensed health professionals.

PERSONAL RESPONSIBILITY AND RELEASE OF HEALTH CARE RELATED CLAIMS

The Client acknowledges that the Client takes full responsibility for the Client’s life and well-being, as well as the lives and well-being of the Client’s family and children (where applicable), and all decisions made during and after this program.

The Client expressly assumes the risks of the Program, including the risks of trying new foods or supplements, and the risks inherent in making lifestyle changes. The Client releases the Coach from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the Client ever had, now has or will have in the future against the Coach, arising from the Client’s past or future participation in, or otherwise with respect to, the Program, unless arising from the gross negligence of the Coach.

CONFIDENTIALITY

The Coach will keep the Client’s information private and will not share the Client’s information to any third party unless compelled to by law.

If the terms of this Agreement are acceptable, please sign the acceptance below. By doing so, the Client acknowledges:

(1) he/she has received a copy of this letter agreement; (2) he/she has had an opportunity to discuss the contents with the Coach and, if desired, have it reviewed by an attorney; and (3) the client understands, accepts and agrees to abide by it.
